For EtG, add-ons, or tests beyond 10 panels, 150mg of the sample is advised. It is best to weigh the sample using a jeweler’s scale. If scalp hair is not available, a similar amount of body hair can be used. By head hair, we mean only scalp hair, whereas body hair includes all other types (such as facial and axillary hair).

Process Overview

The process of lab analysis of a drug test result includes Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first stage of processing a sample into the laboratory’s database. This process involves confirming if the sample was sealed and transported correctly, giving it a unique LAN (Laboratory Accessioning Number), and completing any additional data input not covered by an electronic chain of custody system.

Screening is an initial swift check for drug abuse. While Screening is cost-effective for eliminating drug use in most specimens, a positive result needs confirmation to be used in legal contexts. Any samples that show a positive Screening require a subsequent confirmation.

For samples that show positive in the Screening phase, additional hair is taken from the original specimen for Extraction. This step involves extracting drugs from hair at much lower concentrations than other methods (e.g., urine or oral fluid), making hair drug tests the most challenging to administer.

Confirmation of any positive Screening outcome is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are washed before confirmation if necessary. The comprehensive lab procedure from Accessioning to Confirmation is conducted under both CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
