What it is, How it works

Hair analysis is recognized as an effective method for identifying drug and alcohol consumption. Hair captures a prolonged history of substance use by incorporating biomarkers within the fibers of the hair shaft. When obtained near the scalp, hair can reveal a detection span of up to around three months for drugs and alcohol. The collection process is straightforward, relatively tamper-resistant, and convenient for transportation.

A sample measuring 1.5 inches containing roughly 200 hair strands (comparable to the diameter of a standard pencil) adjacent to the scalp will yield 100mg of hair, which is optimal for initial screening and follow-up confirmation. For EtG, additional analyses, or tests exceeding 10 panels, a specimen mass of 150mg is advisable. Measuring the sample's mass on a jeweler’s scale is suggested. If scalp hair is inaccessible, a comparable quantity of body hair can be obtained. When discussing head hair, it strictly refers to scalp hair. Body hair encompasses all other forms (such as facial and underarm hair).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into the laboratory system. This step verifies that the sample was correctly sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and addresses any additional data entry not covered by an electronic chain of custody system.

Screening is a preliminary assessment for substances of abuse. Although Screening is a cost-efficient method to exclude drug use in the majority of samples, any positive indication requires confirmation to be valid in legal proceedings. Samples that are presumptively positive during Screening require a second confirmation.

For samples deemed presumptively positive in Screening, further hair is extracted from the original specimen and readied for Extraction. This phase involves extracting drugs from hair at much lower concentrations than other techniques (e.g., urine or oral fluid), making hair drug testing the most challenging to conduct.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptively positive samples are cleansed before confirmation as necessary. The complete laboratory process from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair criteria and accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
