What it is, How it works

Hair testing has become recognized as an effective means for identifying drug and alcohol consumption. Hair offers a long-term record of alcohol and drug presence by embedding biomarkers in the fibers of the growing hair shaft. When collected near the scalp, hair can offer a detection window of up to roughly 3 months for alcohol and drug use. Hair collection is straightforward, relatively resistant to tampering, and easy for shipping.

A 1.5-inch sample, consisting of around 200 hair strands (approximate diameter of a #2 pencil), closest to the scalp, provides 100mg of hair, which is the preferred amount for screening and verification. For EtG, add-ons, and/or testing above 10 panels, a sample of 150mg is preferred. We advise weighing the sample with a jeweler’s scale. If scalp hair is not available, an equivalent amount of body hair can be used. By head hair, we mean only scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory workflow for processing a drug test result includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial intake of a specimen into a lab’s system. This step includes ensuring the sample was properly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and performing any additional data entry lacking in an electronic chain of custody system.

Screening involves a preliminary examination for substances of abuse. While Screening is an economical method to exclude drug use in most specimens, a positive result in Screening necessitates verification for court admissibility. Any samples that are preliminary positive in Screening must undergo a secondary confirmation.

If a specimen is initially positive during Screening, additional hair is taken from the original sample and processed for Extraction. In this phase, substances are extracted from hair at much lower levels than with other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to perform.

Confirmation of any positive Screening outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if needed. The comprehensive laboratory process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
