For EtG, additional tests, or tests exceeding 10 panels, 150mg of the sample is advised. We suggest utilizing a jeweler’s scale to weigh the sample. If scalp hair is not available, an equivalent amount of body hair can be used. When mentioning head hair, we refer exclusively to scalp hair. Body hair indicates all other types like facial or axillary hair.

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning starts with the sample's introduction into the laboratory system. This process includes confirming the sample's proper sealing and shipping, assigning an arbitrary LAN (Laboratory Accessioning Number), and completing any additional data input not covered by an electronic chain of custody system.

Screening offers an initial rapid assessment for drug abuse. Although Screening is a budget-friendly way to negate drug usage in the bulk of samples, a positive screen requires confirmation to hold up in legal contexts. Any samples deemed presumptively positive in Screening necessitate further confirmation.

When a sample tests presumptively positive in the Screening phase, additional hair is taken from the initial sample and prepared for Extraction. During this phase, drugs are extracted from hair at a significantly lower concentration than other methods (such as urine or oral fluid), making hair drug screening the most challenging method to execute.

GC/MS, GC/MS/MS, or LC/MS/MS are used to confirm any positive screening outcomes. Prior to confirmation, all presumptive positive samples undergo washing as necessary. The entirety of the laboratory procedure from Accessioning to Confirmation is reviewed under the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
