What it is, How it works

Hair analysis has become recognized as an effective means for identifying drug and alcohol consumption. Hair retains a prolonged history of alcohol and drug intake by capturing biomarkers within the fibers of the growing hair strand. When obtained near the scalp, hair can offer a detection period of up to about 3 months for alcohol and drugs. Hair is easy to obtain, somewhat hard to tamper with, and simple to send.

A 1.5-inch portion comprising roughly 200 hair strands (similar in size to a #2 pencil) closest to the scalp can provide 100mg of hair, which is the ideal amount for screening and confirmation. For EtG, additional testing, and/or tests exceeding 10 panels, 150mg of the sample is suggested. It's advisable to weigh the sample on a jeweler’s scale. If scalp hair is not present, a similar amount of body hair may be collected. When mentioning head hair, we refer specifically to scalp hair. Body hair includes all non-scalp hair types (facial, axillary, etc.).

Process Overview

The primary stages involved in processing a drug test result in a lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial handling of a sample within a lab’s system. This step includes verifying that the sample was properly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and completing any additional information entry not provided by an electronic chain of custody system.

Screening involves an initial rapid check for abusive drugs. While Screening is a cost-effective means to eliminate drug usage in most samples, a positive screen requires confirmation to be valid in court. Any samples that test presumptively positive in Screening need secondary confirmation.

If a sample tests presumptively positive during Screening, additional hair is removed from the original sample and prepared for Extraction. In this phase, drugs are extracted from hair at much lower concentrations than in other methodologies (e.g., urine or oral fluid), making hair drug testing the most challenging methodology to perform.

Confirmation of any positive screening result is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are washed before confirmation as necessary. The entire lab process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
