Remarkably resistant to manipulation due to supervised collection, urine tests, however, remain more reliable for substance use within the past five days.

What it is, How it works

Hair testing is now regarded as an effective method for identifying drug and alcohol consumption. Hair retains a lengthy record of alcohol and drug intake by trapping biomarkers within the strands as they grow. When collected near the scalp, hair can offer a detection period of up to about 3 months for substances. It is easy to gather, relatively tamper-proof, and convenient to transport.

A hair sample of 1.5 inches, consisting of around 200 strands (approximately the size of a #2 pencil) close to the scalp, will yield 100mg of hair, which is ideal for both screening and confirmation. For EtG, add-ons, or tests exceeding 10 panels, 150mg is preferable. We advise using a jeweler’s scale for measurement. If scalp hair is unavailable, a similar quantity of body hair may be used. When we mention head hair, we mean scalp hair only. Body hair encompasses all other types like facial and axillary hair.

Process Overview

The laboratory processing of a drug test outcome involves four crucial stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab's system. This involves ensuring the sample's proper sealing and shipment, assigning it a unique LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody system.

Screening consists of a preliminary inspection for drug abuse. Although Screening is a cost-effective strategy to eliminate drug usage in most samples, court admissibility necessitates confirmation of a positive screen. Samples that are initially positive in Screening require secondary verification.

If a sample presents as presumptively positive at the Screening stage, additional hair is collected from the original sample for Extraction. In this stage, drugs are extracted from hair at significantly lower concentrations than alternative methodologies (like urine or oral fluid), which makes hair drug screening notably challenging.

Confirmation of any positive Screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo necessary washing before confirmation. The whole laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and in alignment with the ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
