What it is, How it works

Hair assessments have become recognized as an effective mechanism for identifying substance use involving drugs and alcohol. Hair captures a prolonged record of alcohol and drugs by encasing biomarkers within the fibers of the hair as it grows. When clipped near the scalp, hair can extend a detection timeframe of up to approximately 3 months for alcohol and drugs. Collecting hair is straightforward, relatively challenging to tamper with, and simple to transport.

A 1.5-inch segment consisting of about 200 hair strands (around the circumference of a #2 pencil) nearest to the scalp yields 100mg, the optimal quantity for analysis and confirmation. For EtG, supplementary tests, and/or screenings beyond 10 panels, a 150mg specimen is advisable. Utilizing a jeweler’s scale is suggested for measuring the sample. If scalp hair is unavailable, a comparable volume of body hair may be gathered. When referencing head hair, we are solely referring to scalp hair. Body hair encompasses all other varieties (facial, axillary, etc.).

Process Overview

The laboratory analysis of a drug test result encompasses four principal stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is initially managed within a lab’s system. This step verifies the sample’s proper sealing and shipping, designates a random LAN (Laboratory Accessioning Number), and finishes any supplementary data input not incorporated through an electronic chain of custody.

Screening involves a preliminary rapid assessment for drugs. Although Screening provides a cost-efficient means to exclude drug presence in most samples, a positive result in Screening necessitates confirmation to be valid in legal proceedings. Each sample with a presumptive positive in Screening requires further confirmation.

Should a sample yield a presumptive positive in Screening, additional hair is extracted from the primary specimen and processed for Extraction. Here, drugs are isolated from hair at much lower levels compared to other methods (such as urine or oral fluid), hence why hair drug screening is notably intricate to perform.

Confirmation of any positive screening is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives undergo washing before confirmation when necessary. The complete laboratory workflow from Accessioning to Confirmation is inspected under both the CAP (College of American Pathologists) Hair categorization and accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
