A sample of 1.5 inches containing about 200 hair strands (similar in size to a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the perfect sample size for analysis and verification. For EtG, enhancements, and/or tests exceeding 10 panels, 150mg of the sample is advisable. We suggest measuring the sample on a jeweler's scale. Should scalp hair be unavailable, a comparable quantity of body hair can be collected. When referring to head hair, it pertains solely to scalp hair. Body hair denotes all other types of hair (facial, axillary, etc.).

Process Overview

The four principal steps in the laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial processing of a sample into a lab’s system. This encompasses verifying that the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and accomplishing any additional data entry not available through an electronic chain of custody system.

Screening entails an initial rapid assessment for drugs of abuse. Though Screening is a cost-efficient method to eliminate drug use in most samples, a positive screen must be confirmed to be defensible in legal proceedings. All samples that are presumptively positive in Screening necessitate a secondary confirmation.

If a sample is presumptively positive in the Screening phase, additional hair is retrieved from the initial sample and readied for Extraction. In this phase, drugs are separated from hair at much lower concentrations compared to other methodologies (e.g., urine or oral fluid), which is why hair drug screening is the most challenging methodology to execute.

Confirmation of any positive screening outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as required. The entire laboratory procedure from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
