A 1.5-inch segment of roughly 200 hair strands (comparable to a #2 pencil) closest to the scalp will provide 100mg of hair, the optimal sample for screening and confirmation. For EtG, additional tests, or panels exceeding 10, 150mg of the specimen is advisable. It's suggested to weigh the specimen using a jeweler’s scale. If scalp hair isn't available, an equivalent quantity of body hair might be used. When discussing head hair, we're specifically referring to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result primarily consists of Accessioning, Screening, Extraction, and Confirmation steps.

Accessioning is the initial procedure of integrating a sample into a laboratory’s system. This encompasses confirming the sample's proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and filling in any missing data not covered by an electronic chain of custody system.

Screening entails a preliminary and swift evaluation for drugs of abuse. Although Screening is an economical method to exclude drug use in the majority of samples, a positive screening result must be verified to hold up in court. Any samples that yield a presumptive positive in Screening necessitate additional confirmation.

Upon a presumptive positive result during the Screening phase, more hair is extracted from the original sample for Extraction. This phase involves extracting drugs from hair at significantly lower concentrations than other methods (such as urine or oral fluid), making hair drug screening the most complex methodology.

Confirmation of a positive screening result is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as necessary. The whole laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
