Hair analysis is increasingly acknowledged as a robust method for identifying drug and alcohol consumption. Hair captures a comprehensive record of alcohol and other substances by embedding biomarkers within the strands as they grow. When extracted near the scalp, hair offers an approximate 3-month timeframe for detecting alcohol and other drugs. The process of collecting hair is straightforward, resistant to tampering, and simple to transport.
A 1.5-inch section consisting of around 200 hair strands (roughly the thickness of a #2 pencil) proximate to the scalp yields 100mg of hair, which is the optimal amount for initial testing and subsequent confirmation. For EtG, extended screens, and/or analyses exceeding 10 panels, a 150mg sample is advocated. We advise using a jeweler’s scale for specimen measurement. In the absence of scalp hair, an equivalent volume of body hair may be utilized. The term head hair refers specifically to scalp hair. Body hair denotes all other hair varieties (facial, axillary, etc.).
Process Overview
The four primary stages in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.
Accessioning entails the preliminary integration of a sample into a lab’s framework. This incorporates confirming the sample’s proper sealing and shipping, generating a random LAN (Laboratory Accessioning Number), and completing supplementary data entry not covered by an electronic chain of custody system.
Screening entails an initial rapid assessment for abused drugs. Although Screening is a cost-effective approach to exclude drug usage in most samples, a positive result requires verification for legal acceptability. Any samples with a presumptive positive result in Screening necessitate further confirmation.
If a sample is presumptively positive during the Screening phase, additional hair is extracted from the original sample and readied for Extraction. In this phase, drugs are isolated from the hair in much lower concentrations than with other techniques (such as urine or oral fluid), which makes hair drug analysis one of the most challenging methodologies to conduct.
Confirmation of any positive Screening result is performed with GC/MS, GC/MS/MS, or LC/MS/MS methods. All presumptive positive samples undergo washing before confirmation as necessary. The entire laboratory sequence from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.
