A sample measuring 1.5 inches and consisting of roughly 200 hair strands (comparable to the thickness of a #2 pencil) near the scalp provides 100mg of hair, which is optimal for both screening and verification. For EtG, additional tests, and/or panels exceeding 10, 150mg of hair is advisable. Using a jeweler’s scale for precision is recommended. If scalp hair cannot be obtained, the equivalent quantity of body hair may be used. When we mention head hair, we mean scalp hair exclusively. Body hair refers to all other types such as facial or underarm hair.

Process Overview

The process for evaluating a drug test sample in a lab comprises four major phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails enrolling a sample in the laboratory system. This phase includes confirming that the sample is properly sealed and transported, generating a random Laboratory Accessioning Number (LAN), and completing additional data entry as needed outside of the electronic chain of custody system.

Screening provides a preliminary and rapid check for drug presence. While cost-efficient for dismissing drug use in most samples, a positive screening must be validated for it to hold up legally. Any samples with a presumptive positive result in Screening require further confirmation.

In cases of a presumptive positive during Screening, additional hair is extracted from the primary sample for the Extraction phase. Here, drugs are isolated from hair at a lower concentrate compared to other methods (e.g., urine or oral fluids), making hair analysis one of the most intricate methods to execute.

Confirming any positive screened result is done using techniques such as GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples go through a washing process before confirmation if necessary. The entire progression from Accessioning to Confirmation is assessed according to the CAP (College of American Pathologists) Hair designation and in compliance with ISO / IEC 17025 standards.
