Hair analysis has become recognized as a potent resource for identifying drug and alcohol consumption. It offers an extended timeline of substance exposure by embedding biomarkers within the strands of hair as they grow. When hair is cut near the scalp, it can supply a detection duration of up to about 90 days for both alcohol and drugs. It is straightforward to gather, somewhat resistant to tampering, and convenient to transport.
A 1.5-inch hair sample consisting of around 200 strands (approximately the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal amount for testing and confirmation. For EtG assessments, additional screenings, and/or tests over 10 panels, 150mg of hair is advisable. It's suggested to weigh the hair using a jeweler’s scale. If scalp hair can't be obtained, a similar quantity of body hair can be collected. "Head hair" refers strictly to scalp hair. "Body hair" encompasses all other types of hair (such as facial, axillary, etc.).
Process Overview
The primary phases in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.
Accessioning starts with the sample being entered into the laboratory’s system. This process ensures the sample was correctly sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and completes any missing data entry not handled by an electronic chain of custody system.
Screening involves a preliminary rapid check for substances of abuse. While it is an economical method to rule out drug presence in most samples, a positive screening requires confirmation to be validated legally. All samples showing a presumptive positive result in Screening require further confirmation.
If a sample shows a presumptive positive result during Screening, additional hair is extracted from the initial sample for Extraction. In this phase, drugs are isolated from hair at lower concentrations relative to other methods (such as urine or oral fluid), making hair drug testing the most challenging approach.
Results from any positive screenings are verified using GC/MS, GC/MS/MS or LC/MS/MS. Presumptive positive samples undergo washing before confirmation if necessary. The laboratory process from Accessioning to Confirmation is inspected under both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 certification.
