A significant advantage is the extended detection window, spanning up to 90 days, combined with the observed collection process that reduces cheating opportunities, although urine tests remain optimal for detecting drug use within the last 5 days.

What it is, How it works

Hair tests have emerged as an effective method for identifying drug and alcohol usage. Hair captures a long-term profile of alcohol and drug exposure by encasing markers within the strands of growing hair. When collected near the scalp, hair can reveal a detection range of up to around 3 months for alcohol and drugs. Hair collection is straightforward, somewhat resistant to tampering, and convenient for transport.

A hair sample measuring 1.5 inches, comprising roughly 200 strands (about the girth of a #2 pencil) nearest the scalp, yields 100mg of hair, ideal for screening and confirmation. For EtG, extended panels, and/or tests beyond 10 panels, 150mg of the sample is needed. We suggest weighing the sample on a jeweler’s scale. If scalp hair is not present, a similar quantity of body hair can be used. By head hair, we specifically mean scalp hair. Body hair encompasses other types (facial, axillary, etc.).

Process Overview

The laboratory processing of drug test outcomes involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial integration of a sample into a lab's system. This step ensures the sample is sealed and shipped correctly, allocates a random LAN (Laboratory Accessioning Number), and fills in additional data entry not covered by an electronic chain of custody.

Screening executes an initial rapid assessment for abuse drugs. Though cost-effective in ruling out drug use for most samples, a positive screen requires confirmation for legal admissibility. All samples that test presumptively positive in Screening must undergo secondary confirmation.

If a sample tests presumptively positive during Screening, additional hair is extracted from the initial sample for Extraction. Here, drugs are isolated from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging technique to implement.

GC/MS, GC/MS/MS, or LC/MS/MS techniques perform Confirmation of any positive screening outcomes. Presumptive positive samples undergo cleansing as necessary before confirmation. The complete lab process from Accessioning to Confirmation undergoes review under CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Furthermore, the SoHT meeting in Verona featured research on the identification and quantification of alcohol abuse biomarkers, their stability, and their correlation with exposure and other biomarkers.

Forensic Applications and Interpretation: One of the vital objectives of hair testing is its application in forensic investigations and legal proceedings. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
