For very recent drug use detection, within 5 days, urine tests remain most precise.

What it is, How it works

Hair analysis is now recognized as an effective method for identifying the use of drugs and alcohol. Hair can capture a comprehensive timeline of alcohol and drugs, as it absorbs biomarkers within the structure of the elongating hair strand. When a sample is taken close to the scalp, it offers a detection period of up to roughly 3 months for substances. Hair is straightforward to obtain, relatively resistant to tampering, and convenient to transport.

A sample measuring 1.5 inches with roughly 200 hair strands (approximately the girth of a #2 pencil) closest to the scalp provides 100mg of hair, perfect for evaluation and validation. For EtG, supplementary tests, and/or assessments over 10 panels, 150mg of the sample is advised. It's ideal to use a jeweler’s scale to measure the sample. In the absence of scalp hair, a similar quantity of body hair can be used. When we mention head hair, we specifically mean scalp hair. Body hair encompasses all other hair varieties (such as facial, underarm, etc.).

Process Overview

The drug test result processing in a laboratory primarily consists of four stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is first processed into the lab’s system. This step includes ensuring the sample was securely sealed and sent, assigning a unique LAN (Laboratory Accessioning Number), and completing any extra data inadequacies not provided by an electronic chain of custody system.

Screening involves a preliminary rapid assessment for prohibited substances. Although Screening is a financially efficient way to eliminate drug usage in the majority of cases, a positive result in Screening requires confirmation for legal admissibility. Any samples that test positive initially in Screening need further verification.

If Screening indicates a positive result, additional hair is taken from the original sample for Extraction. Here, drug compounds are isolated from the hair at far lower levels than other methods (for instance, urine or oral samples), making hair drug testing a challenging method to execute.

Verification of any positive Screening finding is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All preliminary positive samples are rinsed before confirmation if needed. The complete lab process, from Accessioning to Verification, is evaluated under the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
