For EtG, add-on tests, or those exceeding 10 panels, a larger sample of 150mg is advised. Using a jeweler’s scale for specimen weight is suggested. If scalp hair is not available, a comparable amount from body hair can be used. For hair on the head, we specify scalp-derived strands exclusively. Body hair includes all other types (such as facial, axillary, etc.).

Process Overview

The primary phases in processing drug test results in a lab include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the introductory phase where the specimen is enlisted into the lab’s system. This step confirms the sample’s appropriate sealing and shipping, assigns a random LAN (Laboratory Accessioning Number), and concludes any additional data entry absent from an electronic custody system.

Screening offers an initial cursory assessment for drug substances. Though Screening provides a cost-effective method to exclude drug use for most samples, a positive result in this phase must undergo Confirmation for legality. Any samples with a presumptive positive result in Screening must go through subsequent verification.

If Screening yields a presumptive positive result, more hair is extracted from the initial sample for Preparation. Here, drugs are retrieved from hair at considerably lower concentrations compared to other methods (e.g., urine or oral fluids), making hair drug analysis particularly challenging.

Positive screening results are validated through GC/MS, GC/MS/MS, or LC/MS/MS. Any samples considered presumptive positive are cleansed before Confirmation when required. The entire lab procedure from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.
