When conducting tests like EtG or panels beyond 10, a 150mg sample is preferred. A jeweler’s scale is suggested for weighing the specimen. If there's a lack of scalp hair, a comparable quantity of body hair can be used. “Head hair” refers solely to scalp hair, while “body hair” covers other types (facial, axillary, etc.).

Process Overview

The drug test result process in a lab encompasses four essential steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into the lab's system, ensuring it was sealed and sent properly, assigning a LAN (Laboratory Accessioning Number), and completing any data entry missing from the electronic chain of custody system.

Screening is a preliminary drug use assessment. It’s a budget-friendly strategy for ruling out drug use in most samples, but a positive screen must be confirmed for legal admissibility. Samples that show presumptive positivity in Screening need additional confirmation.

In cases of presumptive positivity during Screening, more hair is retrieved from the specimen for Extraction. Here, drugs are extracted at lower concentrations compared to other methods (e.g., urine or oral fluid), which makes hair drug screening particularly challenging.

Confirmation for any positive screening involves GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation as necessary. The entire lab procedure—from Accessioning to Confirmation—adheres to both the CAP (College of American Pathologists) Hair standards and ISO / IEC 17025 accreditation.

The pitfalls encountered when the exogenous/endogenous nature of a substance is questioned were presented and discussed. Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
