When acquired directly from the scalp, hair potentially offers a detection timeframe of up to about 90 days for alcohol and drugs. Collecting hair is straightforward, moderately challenging to tamper with, and facile to transport.

A segment of 1.5 inches from roughly 200 hair strands (comparable to the breadth of a #2 pencil) nearest to the scalp yields 100mg of hair, the optimal quota for testing and verification. For EtG, additional tests, and/or assessments exceeding 10 panels, 150mg of the hair specimen is suggested. Utilizing a jeweler’s scale for measuring the sample is advisable. If scalp hair is inadequate, a similar quantity of body hair can serve as a substitute. The term head hair pertains exclusively to scalp hair. Body hair encompasses all other forms such as facial and axillary.

Process Overview

The laboratory processing of a drug test comprises four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the introductory phase of embedding a sample into a lab’s workflow. This process verifies that the sample’s sealing and shipment are correct, allocates a unique LAN (Laboratory Accessioning Number), and fills in any supplementary data not handled by an electronic chain of custody system.

Screening constitutes an initial rapid verification for substances of abuse. Although Screening effectively eliminates drug consumption on most samples economically, a positive screen necessitates confirmation to be admissible in legal contexts. Samples that test as presumptively positive during Screening must undergo further confirmation.

Should a sample register as presumptively positive during the Screening phase, additional hair is extracted from the original sample and readied for Extraction. This process retrieves drugs from hair at a considerably lower concentration than other techniques (e.g., urine or oral fluid), revealing hair drug screening as one of the most challenging methods to execute.

Confirmation of any positive Screening findings is performed via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo preliminary washing before confirmation if necessary. The entire process from Accessioning to Confirmation receives scrutiny under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The manuscripts in this section presented novel approaches, such as multianalyte methods and on-line preparation which can enable rapid analysis and improve thorough forensic investigations.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
