What it is, How it works

Hair captures a prolonged history of substance use by embedding biomarkers within the fibers of growing hair. When gathered near the scalp, it offers a detection window of up to approximately three months for substances. Collecting hair is straightforward, relatively hard to tamper with, and convenient to transport.

By taking a 1.5-inch segment of roughly 200 hair strands (similar in thickness to a #2 pencil) nearest to the scalp, one can obtain 100mg of hair, which is the optimal amount for both screening and confirmation processes. For EtG, supplementary add-ons, and/or tests exceeding 10 panels, 150mg of the sample is advised. Using a jeweler’s scale for weighing the sample is suggested. If scalp hair is not accessible, an equivalent quantity of body hair can be used. When mentioning head hair, we specifically mean scalp hair, while body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory handling of a drug test result consists of four principal stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where a sample is entered into a laboratory’s system. This procedure includes confirming that the sample was properly sealed and mailed, assigning a unique LAN (Laboratory Accessioning Number), and finishing any supplementary data entry not available in an electronic chain of custody system.

Screening provides a preliminary, rapid assessment for illicit drugs. Although Screening is an economical means to eliminate drug usage in most samples, a positive result necessitates confirmation to be legally valid. All samples that are tentatively positive in Screening require further confirmation.

If a sample tests tentatively positive in the Screening process, additional hair is extracted from the original sample and readied for the Extraction phase. Here, drugs are extracted from the hair at a notably lower concentration than other techniques (e.g., urine or saliva), making hair drug testing the most challenging technique to execute.

Confirmation of any positive finding in the screening phase is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All provisional positive samples undergo washing prior to confirmation if required. The entire laboratory process, from Accessioning to Confirmation, is evaluated under both the CAP (College of American Pathologists) Hair accreditation and the ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
