Its collection process is straightforward, tampering is somewhat tricky, and transportation is uncomplicated.

A 1.5-inch portion consisting of roughly 200 hair strands (about the diameter of a #2 pencil) nearest the scalp yields 100mg of hair, which is optimal for verification. For EtG, additional screening, and/or assessments over 10 panels, a 150mg sample is advisable. Using a jeweler’s scale to weigh the specimen is suggested. If scalp hair isn't accessible, a similar quantity of body hair can be taken. By head hair, we specify scalp hair exclusively. 'Body hair' pertains to all other types (facial, axillary, etc.).

Process Overview

The laboratory process of a drug test consists of four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the sample's preliminary entry into the lab’s system. This step ensures the sample was appropriately sealed and sent, assigns a random LAN (Laboratory Accessioning Number), and involves completing any missing data entry if an electronic chain of custody doesn't provide it.

Screening is a preliminary rapid test for drug presence. Though it's a budget-friendly approach to rule out drugs in most cases, a positive screen must undergo confirmation for judicial acceptance. Samples testing positive in Screening necessitate an additional confirmation step.

If Screening reveals presumptive positivity, more hair is extracted from the original sample and prepped for Extraction. In this phase, substances are isolated from hair at considerably lower concentrations than other techniques (e.g., urine or saliva), making hair analysis an intricate methodology.

Using GC/MS, GC/MS/MS, or LC/MS/MS, any positive screening outcomes are authenticated. Prior to confirmation, all presumptive positive samples are cleansed as needed. Every laboratory stage from Accessioning to Confirmation is audited according to both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
