Process Overview

This step includes verifying the sample's proper sealing and shipping, assigning a unique Laboratory Accessioning Number (LAN), and completing any supplementary data entry not available via an electronic chain of custody system.

Screening is a preliminary rapid assessment for drugs of abuse. Although Screening is a cost-efficient strategy to dismiss drug use in the majority of samples, positive screens require confirmation to be accepted in legal settings. Presumptively positive samples during Screening necessitate a secondary confirmation.

If a sample shows presumptive positivity in the Screening stage, additional hair is extracted from the original specimen and readied for Extraction. In this phase, drugs are isolated from hair at much lower concentrations compared to other methods (such as urine or oral fluid), making hair drug screening the most challenging to execute.

For confirming any positive screening outcome, techniques like GC/MS, GC/MS/MS, or LC/MS/MS are utilized. All presumptive positives undergo a washing before confirmation, as needed. The comprehensive laboratory procedure from Accessioning through to Confirmation is evaluated according to both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation requirements.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.
