Process Overview

The processing of a drug test result in the laboratory encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning denotes the initial incorporation of the sample into a lab’s system by ensuring proper sealing and shipping of the sample, as well as assigning it an arbitrary LAN (Laboratory Accessioning Number) just after any extra data entry tasks not managed by an electronic chain of custody system.

Any samples with presumptive positive results during Screening must undergo further confirmation.

At this point, extraction of drugs from hair happens at far lower concentrations compared to other methods (e.g., urine or oral fluid), highlighting the complexity of hair drug screening.

To validate any positive results from Screening, it is confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all samples indicating presumptive positivity are cleansed where necessary. The entire process, spanning from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair designation and adheres to ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
