For EtG, additional tests, and over 10-panel screenings, 150mg of hair is advisable. It is recommended to weigh the sample using a jeweler’s scale. If scalp hair is unavailable, an equivalent amount of body hair may be used. When mentioning head hair, we specifically refer to scalp hair only. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The four primary phases involved in the laboratory analysis of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial sample processing into the laboratory’s system. This process includes ensuring the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any outstanding data entry not covered by an electronic chain of custody system.

Screening involves a preliminary rapid check for substance abuse. Screening is an economical method to rule out drug use for most samples; however, a positive screening requires confirmation to be legally admissible. Any samples presumptively positive during Screening necessitate additional confirmation.

If a sample indicates a presumptive positive in the Screening phase, more hair is extracted from the initial specimen and processed for Extraction. During this step, drugs are extracted from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), which is why hair drug screening is the most complex methodology to execute.

Confirmation of any positive screening outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are cleansed prior to confirmation when necessary. The entire laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair certification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
