For EtG, additional checks, and/or tests exceeding 10 panels, it is suggested to use 150mg of hair. It is advisable to weigh the sample using a jeweler’s scale. Should scalp hair be unavailable, a similar quantity of body hair can be gathered. References to head hair pertain solely to scalp hair, while body hair denotes all other types (such as facial or axillary hair).

Process Overview

The laboratory processing of drug test results comprises four main stages: Accessioning, Screening, Extraction, and Confirmation.

In Accessioning, the sample is initially processed into the laboratory system. This step includes ensuring the sample was properly sealed and transported, assigning it a random LAN (Laboratory Accessioning Number), and completing any necessary data entry not covered by an electronic chain of custody.

Screening is a preliminary check for drugs of abuse. Although Screening is a cost-effective method to eliminate drug use in most samples, a positive screening result must be confirmed for it to be admissible in legal contexts. Samples that are initially positive in Screening need a follow-up confirmation.

If Screening indicates a potential positive, additional hair is extracted from the original specimen for the Extraction phase. Here, drugs are retrieved from hair at considerably lower concentrations than those in other testing methods (e.g., urine or oral fluid), explaining why hair drug screening is the most challenging technique to execute.

Positive screening outcomes proceed to Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS. All preliminary positive samples are cleansed as necessary before confirmation. The entire procedure, from Accessioning to Confirmation, is subject to review under the CAP (College of American Pathologists) Hair standards and is accredited to ISO / IEC 17025 regulations.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
