What it is, How it works

When sampled near the scalp, hair may offer a detection window of up to roughly 3 months for both alcohol and drugs. Hair collection is straightforward, resistant to tampering, and convenient for transport.

A 1.5-inch segment consisting of around 200 strands of hair (roughly the diameter of a #2 pencil) closest to the scalp yields 100mg of hair, the optimal sample size for both screening and confirmation. For EtG, supplemental, and/or tests exceeding 10 panels, a specimen weight of 150mg is advisable. We suggest weighing the specimen on a jeweler’s scale. In cases where scalp hair is lacking, an equivalent volume of body hair may be collected. References to head hair pertain solely to scalp hair. Body hair refers to other hair varieties (facial, axillary, etc.).

Process Summary

The key phases in the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab’s system. This involves ensuring the sample was properly sealed and dispatched, assigning an arbitrary LAN (Laboratory Accessioning Number), and completing any supplementary data input not captured by an electronic chain of custody mechanism.

Screening entails a preliminary rapid assessment for substances of abuse. Though Screening is an economical means to rule out drug use in most samples, a positive result needs affirmation to be acceptable in legal proceedings. Any samples showing presumed positivity in Screening necessitate secondary verification.

Should a sample indicate presumed positivity during the Screening phase, additional hair is extracted from the original specimen and prepped for Extraction. During this phase, drugs are extracted from hair at significantly lower concentrations than in other methodologies (e.g., urine or oral fluid), making hair drug analysis the most challenging approach to execute.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumed positive samples undergo washing before confirmation as required. The full laboratory sequence from Accessioning to Confirmation is overseen under both CAP (College of American Pathologists) Hair criteria and the accreditation to ISO / IEC 17025 standards.
