What it is, How it works

A 1.5-inch section of roughly 200 hair strands (comparable to a #2 pencil) located nearest to the scalp yields 100mg of hair, which is optimal for both screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, a specimen weighing 150mg is advisable. It is suggested to measure the specimen with a jeweler’s scale. If scalp hair isn't available, a similar volume of body hair can be used. The term head hair denotes scalp hair exclusively, while body hair covers all other types (such as facial, underarm, etc.).

Process Overview

The essential phases involved in processing a drug test result in a lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the initial entry of a specimen into a lab's system. This step checks that the specimen was appropriately sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and completes any absent data entry not supplied by an electronic chain of custody system.

Screening offers a preliminary rapid evaluation for substances of abuse. Although Screening is a cost-efficient means to rule out drug use in most specimens, a positive screen must be verified to be legally valid. Specimens that show presumptive positive results during Screening require a subsequent confirmation.

If a specimen tests positive during the Screening stage, more hair is taken from the initial sample to prepare for Extraction. In this phase, substances are isolated from hair at a much lower concentration than in other methods (such as urine or oral fluid), making hair drug screening the most challenging method to conduct.

Confirmation of any positive screening result is carried out via GC/MS, GC/MS/MS, or LC/MS/MS. All samples showing presumptive positives are cleansed before confirmation as necessary. The complete laboratory workflow from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and in accordance with ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
