When sampled near the scalp, hair can reveal drug or alcohol use within approximately a 90-day timeframe. Hair collection is straightforward, challenging to tamper with, and simple to transport.

A hair sample measuring 1.5 inches, consisting of about 200 strands (equivalent to the thickness of a #2 pencil) near the scalp, yields 100mg, which is ideal for testing and verification. For EtG, additional tests, and screens beyond 10 panels, a 150mg sample is suggested. It's advisable to weigh the hair on a jeweler's scale. If scalp hair isn't accessible, an equal quantity of body hair may be used. When discussing head hair, we specifically mean scalp hair. Body hair encompasses all other types (facial, axillary, and others).

Process Overview

The laboratory processing of a drug test follows four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step, where a sample is incorporated into a lab's system. This involves checking that the sample is properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody system.

Screening is a preliminary evaluation for substance abuse. Although Screening is a cost-efficient method to eliminate drug use for most samples, a positive result must be validated to be accepted in a court setting. Any samples that test presumptively positive in Screening require further confirmation.

If a sample shows a presumptive positive during Screening, additional hair is extracted from the original sample for Extraction. In this phase, substances are separated from hair at a considerably lower concentration than other methods (such as urine or saliva), making hair testing the most challenging method to execute.

Confirmation of any positive screen is done through GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are cleansed prior to confirmation as required. The whole lab process from Accessioning to Confirmation is reviewed under the CAP (College of American Pathologists) Hair guidelines and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
