What it is, How it works

When hair is cut near the scalp, it can reveal approximately three months' worth of drug and alcohol history. This method is straightforward to collect, challenging to tamper with, and convenient to transport.

To obtain an optimal sample for both initial screening and subsequent confirmation, a 1.5-inch segment consisting of roughly 200 hair strands (comparable in size to a #2 pencil) closest to the scalp is recommended, yielding about 100mg of hair. For tests exceeding 10 panels or including EtG add-ons, it's advised to collect 150mg of the sample. Utilizing a jeweler's scale for weighing is recommended. If scalp hair is not available, an equivalent quantity of body hair may be used. References to head hair pertain specifically to scalp hair, while body hair encompasses all other types (like facial or axillary hair).

Process Overview

Laboratory drug testing typically involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

In Accessioning, the sample undergoes an initial process to integrate it into the laboratory's system. This includes ensuring proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not captured by an electronic chain of custody system.

Screening provides an initial fast assessment for substances of abuse. Although Screening serves as a cost-efficient means to dismiss drug use in most samples, a positive result in this phase needs validation to be legally permissible. Any samples that test presumptively positive during Screening require further confirmation.

Samples testing presumptively positive during Screening have more hair extracted from the original sample for the Extraction phase. In this phase, hair drug screening faces the challenge of extracting drugs from hair at lower concentrations compared to other methods (e.g., urine or oral fluid).

Confirmation for any positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptive positive samples are cleansed prior to confirmation, if required. The entire lab process from Accessioning to Confirmation is subject to review under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
