For EtG, additional tests, and/or those beyond 10 panels, 150mg of the specimen is advised. It is suggested to weigh the specimen on a jeweler’s scale. Should scalp hair be unavailable, a comparable quantity of body hair can be gathered. When mentioning head hair, it pertains solely to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

There are four primary steps in the laboratory processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the initial integration of a sample into a laboratory’s system. This encompasses confirming the proper sealing and shipping of the sample, allocating a random LAN (Laboratory Accessioning Number), and performing any supplementary data entry beyond what is provided by an electronic chain of custody system.

Screening is a preliminary rapid assessment for drugs of abuse. Although Screening is a cost-efficient approach to exclude drug usage in most samples, confirmation is necessary for positive screens to be court-admissible. Any samples that test presumptively positive during Screening necessitate a follow-up confirmation.

Should a sample test presumptively positive in Screening, further hair is extracted from the initial specimen and readied for Extraction. At this stage, drugs are drawn out from hair at a significantly lower concentration compared to other methodologies (e.g., urine or oral fluid), rendering hair drug screening a notably challenging methodology to execute.

Validation of any positive screening result is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as required. The complete laboratory process from Accessioning to Confirmation is conducted under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
