Collecting hair is straightforward, somewhat challenging to tamper with, and convenient to transport.

A 1.5-inch piece comprising around 200 strands of hair (roughly equal to the width of a #2 pencil) taken nearest the scalp will yield 100mg of hair, the perfect sample size for screening and confirming results. For EtG, extra tests, and/or panels exceeding 10, a sample size of 150mg is advised. We suggest measuring the sample with a jeweler’s scale. If scalp hair is absent, an equivalent volume of body hair can be collected. References to head hair specify scalp hair exclusively. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The lab processing of drug test outcomes is composed of four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase of entering a sample into a lab's system. This step includes confirming that the sample was sealed and dispatched correctly, allocating a random LAN (Laboratory Accessioning Number), and fulfilling additional data entry requirements not covered by an electronic chain of custody system.

Screening consists of an initial rapid drug abuse check. While Screening is an economical approach to eliminating drug usage in most samples, results must be confirmed to be court-admissible. Any sample testing presumptive positive in Screening must undergo further confirmation.

If a sample tests presumptive positive during Screening, additional hair is extracted from the original sample and readied for Extraction. Here, drugs are removed from hair at a concentration much lower than alternative methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging technique to execute.

Confirmation of positive screening outcomes is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens are cleansed before confirmation when necessary. The entire laboratory process from Accessioning to Confirmation undergoes scrutiny under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
