What it is, How it works

Hair collection is straightforward, relatively resistant to tampering, and convenient for transport.

A sample measuring 1.5 inches and consisting of roughly 200 hair strands (about the thickness of a #2 pencil) closest to the scalp will yield 100mg, which is the ideal portion for both screening and confirmation purposes. For EtG, additional tests, and/or panels exceeding 10, a specimen of 150mg is advised. It's recommended to measure the specimen using a jeweler's scale. If scalp hair is insufficient, the same quantity of body hair may be used. When mentioning head hair, it specifically references scalp hair. In contrast, body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial sample integration process into a lab's system. It involves ensuring the sample was properly sealed and sent, allocating a random LAN (Laboratory Accessioning Number), and finalizing any supplementary data entry that isn’t available via an electronic chain of custody system.

Screening consists of a preliminary swift inspection for drug abuse. Although Screening is economical for eliminating drug use in most samples, a positive result requires confirmation for court acceptability. Any samples that give a preliminary positive in Screening need additional confirmation.

If a sample gives an initial positive during Screening, extra hair is extracted from the original specimen to prepare for Extraction. In this phase, drugs are extracted from hair at significantly lower concentrations than other methodologies (e.g., urine or oral fluid), rendering hair drug screening the most challenging methodology to execute.

Confirmation on any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleaned as necessary prior to confirmation. The entire lab process, from Accessioning to Confirmation, is conducted under both the CAP (College of American Pathologists) Hair designation and in accordance with ISO / IEC 17025 standards.
