A 1.5-inch segment of about 200 hair strands (approximately the girth of a #2 pencil) nearest the scalp provides 100mg of hair, which is the optimal sample for analysis and validation. For EtG, additional tests, and/or examinations beyond 10 panels, a 150mg sample is advised. We suggest measuring the specimen using a jeweler’s scale. If scalp hair is inaccessible, an equivalent amount of body hair can be used. When discussing head hair, we mean scalp hair solely. Body hair pertains to any other hair types (facial, axillary, etc.).

Process Overview

The four key stages in laboratory handling of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary processing of a sample into a laboratory's database. This task encompasses verifying the sample was sealed and dispatched correctly, assigning a unique LAN (Laboratory Accessioning Number), and finishing any supplementary data entry not handled through an electronic chain of custody method.

Screening entails an initial rapid assessment for substances of abuse. While Screening is an economical way to rule out drug use in the majority of cases, a positive result requires confirmation to stand in legal settings. Any samples that appear presumptively positive in Screening need a secondary confirmation.

If a sample shows presumptively positive during Screening, more hair is extracted from the original specimen and prepared for Extraction. At this phase, drugs are drawn from hair at significantly lower concentrations than in other processes (such as urine or saliva), making hair drug screening the most challenging method to execute.

Verification of any positive screening outcome is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if necessary. The entire laboratory sequence from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
