A 1.5-inch portion of around 200 hair strands (similar to the thickness of a #2 pencil) close to the scalp yields 100mg of hair, which constitutes the optimal sample for screening and confirmation. For EtG, additional tests, and/or tests above 10 panels, 150mg of the specimen is advisable. We advocate for weighing the specimen on a jeweler’s scale. If scalp hair is unavailable, an equal volume of body hair can be collected. When referring to head hair, we are exclusively mentioning scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four core steps in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial introduction of a sample into a laboratory’s system. This process includes confirming that the sample was sealed and transported correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not covered by an electronic chain of custody system.

Screening provides an initial quick assessment for drugs of abuse. While Screening is a cost-efficient method to eliminate drug usage on the majority of samples, a positive screen must be substantiated to be admissible in court. Any samples that are presumptively positive in Screening necessitate a secondary confirmation.

If a sample is presumptively positive in the Screening phase, more hair is extracted from the original specimen and prepped for Extraction. In this phase, drugs are extracted from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to undertake.

Any positive screening result is confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. All preliminary positive samples undergo a wash prior to confirmation as necessary. The complete laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.
