For EtG, add-ons, and tests exceeding 10 panels, 150mg of the sample is advised. It is suggested to measure the sample using a jeweler’s scale. If scalp hair is not available, a comparable quantity of body hair can be used. When mentioning head hair, we specifically mean scalp hair. Body hair denotes any other types (such as facial, axillary, etc.).

Process Overview

The principal stages in processing a drug test result in a laboratory are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial entry of a sample into the laboratory’s database. This step confirms the sample was sealed and transported correctly, assigns a unique LAN (Laboratory Accessioning Number), and completes any necessary data entry not covered by an electronic chain of custody system.

Screening is an initial rapid assessment for drugs of abuse. While Screening is an economical approach to exclude drug presence in most samples, a positive screening result requires confirmation to be valid in legal proceedings. Any samples presumed positive in Screening must undergo secondary confirmation.

If a Screening results in a presumptive positive, additional hair from the initial sample is prepared for Extraction. In this phase, drugs are isolated from hair at a much lower concentration compared to other methods (e.g., urine or oral fluids), underscoring the complexity of hair drug screening.

Confirmation of any presumptive positive screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The complete laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair criteria and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
