What it is, How it works

Hair analysis has emerged as a potent method for identifying drug and alcohol consumption. Hair captures a prolonged record of alcohol and other substances by embedding biomarkers within the fibers of the developing hair strand. When hair is gathered close to the scalp, it can offer a detection timeframe of up to roughly 3 months for alcohol and other substances. Gathering hair is straightforward, somewhat challenging to tamper with, and convenient for shipping.

A 1.5-inch segment consisting of around 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal sample for both screening and confirmation. For EtG evaluations, additional tests, and/or those above 10 panels, 150mg of the sample is advisable. We suggest measuring the sample using a jeweler’s scale. If there's an absence of scalp hair, a comparable quantity of body hair may be collected. When referencing head hair, we're specifically referring to scalp hair. Body hair refers to all other types (facial, axillary, etc.).

Process Overview

The process in the laboratory for determining a drug test outcome includes four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary processing of a specimen into a laboratory’s database. This step encompasses verifying that the specimen was correctly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not included by an electronic chain of custody framework.

Screening includes a preliminary rapid assessment for illicit substances. Although Screening is cost-efficient for dismissing drug usage in most samples, any positive screen must be verified for legal admissibility. Samples indicative of a positive result in Screening mandate a secondary confirmation.

If Screening suggests a positive result for a sample, additional hair is extracted from the original specimen in preparation for Extraction. During this phase, substances are isolated from hair at significantly lower concentrations than in alternative methods (e.g., urine or oral fluid), which is why hair drug screening is considered the most intricate methodology.

Confirmation of any positive screening finding is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive undergo washing prior to confirmation as necessary. The complete laboratory procedure from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair guideline and accreditation to ISO / IEC 17025 benchmarks.

Advantages of hair drug testing:

  • Extended detection timeframe: Hair analysis can reveal drug use for up to 90 days, unlike urine tests that have a more limited detection period.
  • Challenging to tamper with: It is highly challenging to falsify a hair drug test, ensuring greater accuracy in the results.
  • Offers historical insights: It can illustrate a timeline of substance usage over a duration, rather than merely current usage.

Limitations:

  • Inability to detect recent usage: It requires approximately 5-7 days for substances to become detectable in hair.
  • Expense: Hair drug evaluations typically cost more than other drug testing approaches.
  • Variation in outcomes: Elements such as hair color and individual variations in hair growth can influence the concentration of drug metabolites in hair.

Note: While commonly called "hair follicle tests", the procedure actually analyzes the hair strand rather than the follicle beneath the scalp.
