What it is, How it works

Hair testing has emerged as an effective approach for identifying drug and alcohol consumption. Hair captures a long-term record of substances by embedding biomarkers within the fibers of the growing strand. When collected near the scalp, hair can reveal up to about a 3-month detection period for alcohol and other drugs. Hair is easy to collect, somewhat resistant to tampering, and readily shippable.

A sample measuring 1.5 inches with approximately 200 hair strands (roughly the diameter of a #2 pencil) closest to the scalp will yield 100mg of hair, which is ideal for screening and confirmation. For EtG, add-ons, and/or assessments over 10 panels, 150mg of the specimen is advised. Using a jeweler’s scale for measuring the sample weight is recommended. In the absence of scalp hair, an equivalent quantity of body hair may be collected. Reference to head hair implies scalp hair only. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

There are four primary steps in the lab processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial processing of a specimen into a laboratory’s system. This step verifies that the specimen was sealed and shipped accurately, allocates a random LAN (Laboratory Accessioning Number), and completes any additional data entry absent in an electronic chain of custody system.

Screening delivers an initial quick drug abuse assessment. Screening is efficient in ruling out drug use in most specimens, though a positive screen requires confirmation to be court-admissible. Any specimens testing presumptively positive in Screening demand secondary confirmation.

If a sample tests presumptively positive in Screening, additional hair is extracted from the initial sample for Extraction. Here, drugs are extracted at significantly lower concentrations than other techniques (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening outcome occurs via GC/MS, GC/MS/MS or LC/MS/MS. Prior to confirmation, all presumptive positive samples undergo washing as needed. The entire lab process, from Accessioning to Confirmation, undergoes review under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
