What it is, How it works

A 1.5-inch sample consisting of around 200 hair strands (similar in size to a #2 pencil) taken nearest to the scalp yields 100mg of hair, which is the optimal sample for both screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the specimen is advisable. It's suggested to measure the specimen on a jeweler’s scale. In cases where scalp hair isn't available, an equivalent portion of body hair may be gathered. "Head hair" refers exclusively to scalp hair, while "body hair" encompasses all other types such as facial and axillary hair.

Process Overview

The principal stages in the laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary step of introducing a sample into a lab's system. It involves checking the sample's seal and proper shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry that isn't included in an electronic chain of custody system.

Screening serves as an initial swift assessment for possible drug abuse. Though Screening is an economical strategy for excluding drug usage in most samples, any positive findings need confirmation for them to be valid in legal scenarios. Samples that are presumptively positive in Screening require additional confirmation.

For samples testing positive in the Screening phase, additional hair is extracted from the original specimen and prepared for Extraction. In this process, drugs are extracted from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), which makes hair drug screening a more challenging methodology.

Confirmation of a positive screening result is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with positive results are washed as necessary before confirmation. The entire lab process, from Accessioning to Confirmation, is reviewed under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
