A 1.5-inch segment of around 200 hairs (comparable to the width of a #2 pencil) nearest the scalp will yield 100mg of hair—the optimal amount for examination and verification. For EtG, supplementary analyses, or tests exceeding 10 panels, 150mg of the sample is advised. We suggest measuring the sample on a jeweler’s scale. In the absence of scalp hair, a similar quantity of body hair may be used. When we mention head hair, we mean solely scalp hair. Body hair includes all other types (such as facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result primarily involves four steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of entering a sample into a laboratory’s system. This step ensures the sample was properly sealed and dispatched, assigns a unique LAN (Laboratory Accessioning Number), and finalizes any data input not completed by an electronic chain of custody system.

Screening offers a preliminary assessment for substances of abuse. While Screening is economical for excluding drug use in most samples, a positive result requires confirmation for judicial admissibility. Samples with a presumptive positive in Screening must undergo further verification.

For samples with a presumptive positive at the Screening phase, additional hair is extracted from the original sample for the Extraction process. Here, substances are removed from hair at concentrations lower than other methods like urine or oral fluid, making hair drug screening the most challenging testing method.

Confirmation of any positive screen is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed before confirmation as necessary. The entire laboratory sequence from Accessioning to Confirmation is audited under CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.
