When taken from near the scalp, hair offers up to a 3-month detection period for alcohol and other substances. Hair is straightforward to collect, relatively resistant to tampering, and simple to transport.

A 1.5-inch segment of approximately 200 hair strands (comparable to the thickness of a #2 pencil) closest to the scalp provides 100mg of hair, which is the optimal quantity for screening and confirmation processes. For EtG, additional tests, and/or panels exceeding 10, a 150mg sample is advised. It is suggested to use a jeweler’s scale to measure the specimen. If scalp hair cannot be obtained, a similar volume of body hair may be used. References to head hair signify scalp hair exclusively. Body hair encompasses all other hair types (e.g., facial, axillary, etc.).

Process Overview

The key steps in the laboratory handling of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial integration of a specimen into a laboratory’s database. This step comprises ensuring the specimen was sealed and dispatched correctly, assigning a random Laboratory Accessioning Number (LAN), and performing any additional data entry not covered by an electronic chain of custody system.

Screening consists of an initial rapid assessment for substances of abuse. Although Screening is cost-efficient for eliminating drug usage in most samples, a positive result must be confirmed for court acceptance. Any samples with presumptive positive results in Screening need a follow-up confirmation.

Should a sample show a presumptive positive in the Screening phase, more hair is extracted from the original specimen and prepared for Extraction. During Extraction, substances are isolated from hair at a significantly lower concentration compared to other techniques (e.g., urine or oral fluid), hence hair drug screening is considered the most challenging technique to undertake.

Confirmation of any positive screening result is performed via GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples undergo washing before confirmation where necessary. The comprehensive laboratory procedure from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair category and accredited to ISO / IEC 17025 standards.
