What it is, How it works

When collected near the scalp, it offers nearly a 3-month detection range for drugs and alcohol. Hair collection is straightforward, relatively tamper-resistant, and simple to transport.

A 1.5-inch segment of roughly 200 strands of hair (equivalent to the thickness of a #2 pencil) closest to the scalp provides 100mg of hair, which is an optimal sample size for both screening and confirmation. For EtG, add-ons, and/or tests that exceed 10 panels, a 150mg specimen is advised. It is recommended to weigh the specimen using a jeweler’s scale. If scalp hair is not available, a similar amount of body hair serves as an alternative. When mentioning head hair, it specifically indicates scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The primary stages of laboratory drug test result processing include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial integration of a sample into a lab’s system. This process verifies that the sample was properly sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and finishes any supplementary data entry missing from an electronic chain of custody system.

Screening executes an initial rapid assessment for drugs of abuse. Although Screening is a cost-efficient method to exclude drug use in most samples, a positive screen must be verified to be legally admissible. Samples that test presumptively positive during Screening require further confirmation.

If a sample tests as presumptively positive during Screening, more hair is extracted from the initial sample for preparation in the Extraction phase. At this point, drugs are separated from hair at lower concentrations compared to other methodologies (e.g., urine or oral fluid), making hair drug screening one of the most challenging testing methods.

Confirmation of any positive screening result is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo washing before confirmation as necessary. The complete lab process, from Accessioning to Confirmation, is reviewed under the CAP (College of American Pathologists) Hair accreditation and ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
