Collected near the scalp, hair can offer up to approximately a 3-month detection window for substances. Hair collection is straightforward, relatively hard to tamper with, and convenient to transport.

A 1.5-inch segment of approximately 200 hair strands (about the diameter of a #2 pencil) nearest to the scalp provides 100mg of hair, which is the optimal sample size for analysis and verification. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of hair is suggested. We advise weighing the specimen with a jeweler’s scale. If scalp hair is not accessible, an equivalent amount of body hair may be gathered. When discussing head hair, it refers solely to scalp hair. Body hair encompasses all other hair (facial, axillary, etc.).

Process Overview

Lab processing of a drug test result comprises four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage of integrating a sample into a lab's system. This step includes verifying the sample's appropriate sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any further data input not provided by an electronic chain of custody system.

Screening is an initial rapid assessment for the presence of drugs. While Screening is a cost-conscious method for excluding drug use in most samples, a positive result requires confirmation to be legally admissible. Any samples that are preliminarily positive in Screening necessitate secondary confirmation.

If a sample tests preliminarily positive during Screening, more hair is extracted from the initial specimen and readied for Extraction. In this stage, drugs are removed from hair at concentrations much lower than other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of any positive screening outcome is conducted through GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptive positive samples are washed before confirmation as necessary. The entire lab process from Accessioning to Confirmation is assessed by both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
