What it is, How it works

Hair testing has emerged as an effective tool for identifying drug and alcohol consumption. Hair captures a long-term history of substances by embedding biomarkers within the fibers of the continuously growing hair strand. When acquired close to the scalp, hair allows for approximately a 3-month detection period for alcohol and other substances. The collection of hair is straightforward, moderately tough to tamper with, and straightforward to transport.

A 1.5-inch section comprising about 200 strands of hair (approximately the width of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the perfect sample for initial analysis and confirmation. For EtG, additional tests, or tests beyond 10 panels, 150mg of the sample is advised. It is recommended to weigh the specimen using a jeweler’s scale. If scalp hair is not an option, a similar amount of body hair may be taken. In terms of head hair, we specifically mean scalp hair only. Body hair denotes all other hair types (facial, axillary, etc.).

Process Overview

The four fundamental stages involved in the laboratory analysis of a drug test are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary handling of a sample in a laboratory’s system. This step involves verifying that the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and finalizing any additional data entry absent from an electronic chain of custody system.

Screening entails a rapid initial assessment for substances of abuse. While Screening is a cost-efficient method to eliminate drug use across the majority of samples, a positive screen requires confirmation to be admissible in legal settings. Any samples that return a preliminary positive during Screening necessitate a follow-up confirmation.

If a sample shows presumptive positivity during the Screening process, further hair is extracted from the initial collection and prepped for Extraction. At this stage, substances are removed from the hair at significantly lower concentrations than other techniques (e.g., urine or oral fluid), making hair drug testing the most challenging methodology to execute.

Confirmation of any positive screening results is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation when required. The complete laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
