Process Overview
The drug test laboratory process comprises four essential steps: Accessioning, Screening, Extraction, and Confirmation.
During Accessioning, the sample undergoes initial processing into the laboratory database. This step ensures the sample's intact shipment and sealing, assigns a unique LAN (Laboratory Accessioning Number), and includes data entry beyond what the electronic chain of custody system provides.
Screening performs a preliminary drug abuse check. While cost-effective for negating drug use in most samples, a positive screen mandates confirmation for legal validity. Any samples testing presumptively positive in Screening must undergo secondary confirmation.
If a sample appears presumptively positive in Screening, additional hair is drawn from the specimen for Extraction preparation. This procedure extracts drugs from hair at lower concentrations compared to other tests (e.g., urine or oral fluid), rendering hair drug screening the most complex to implement.
Positive screening results are confirmed via GC/MS, GC/MS/MS, or LC/MS/MS. Samples testing presumptive positive receive a pre-confirmation wash if necessary. Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.
The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
