What it is, How it works

When harvested near the scalp, hair can furnish approximately a 3-month detection range for alcohol and drugs. Hair is straightforward to gather, challenging to tamper with, and convenient to transport.

A sample measuring 1.5 inches of approximately 200 hair strands (comparable to a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal sample size for testing and verification. For EtG, additional tests, and/or panels exceeding 10, 150mg of the specimen is advised. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair is not accessible, an equivalent quantity of body hair might be collected. When indicating head hair, we mean scalp hair exclusively. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The four principal phases in the laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial handling of a sample into a lab’s framework. This entails validating that the sample was correctly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any supplemental data entry absent in an electronic chain of custody system.

Screening constitutes an initial prompt evaluation for drugs of abuse. Although Screening is a cost-efficient method to eliminate drug usage in most samples, a positive screen must be confirmed for court admissibility. Any samples that test presumptive positive in Screening necessitate a secondary confirmation.

If a sample tests presumptive positive in the Screening phase, additional hair is extracted from the initial specimen and prepped for Extraction. In this phase, drugs are extracted from hair at a significantly lower concentration than other methods (e.g., urine or oral fluid), rendering hair drug screening the most challenging methodology to execute.

Any positive screen confirmation occurs via GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation as required. The entire laboratory process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
