For tests involving EtG, additional parameters, or exceeding 10 panels, a 150mg sample is advised. We suggest using a jeweler's scale to measure the specimen. If head hair is inaccessible, a comparable volume of body hair might be used. In this context, 'head hair' exclusively refers to scalp hair, while 'body hair' encompasses all other types (such as facial or axillary).

Process Overview

The lab procedures for processing drug test results involve four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step, where the sample is incorporated into the lab's system. This includes ensuring the sample was properly sealed and transported, generating a random LAN (Laboratory Accessioning Number), and entering any remaining data that isn't automatically provided by an electronic chain of custody.

During Screening, a rapid assessment is conducted to check for substances. While Screening serves as an economical method to dismiss drug use in most cases, a positive screen necessitates further confirmation for legal acceptability. Samples that might test positive at this stage must undergo Confirmation.

If the Screening indicates a tentative positive, additional hair is taken from the original sample to be prepped for Extraction. At this stage, substances are retrieved from hair, albeit at a lower concentration compared to other methods (such as urine or saliva tests), highlighting the challenges of hair drug screening methodology.

The Confirmation of a preliminary positive outcome is done using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples that test positive initially undergo washing before confirmation. The full lab process, from Accessioning to Confirmation, is conducted under CAP (College of American Pathologists) Hair designation and adheres to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
