What it is, How it works

When taken from near the scalp, hair can reveal drug and alcohol use for up to 3 months. Hair is easy to collect, relatively resistant to tampering, and simple to send.

A 1.5-inch segment of roughly 200 hair strands (comparable to a #2 pencil thickness) closest to the scalp yields 100mg of hair, which is the optimal quantity for examination and verification. For EtG, add-ons, and/or investigations exceeding 10 panels, 150mg of hair is advisable. We suggest using a jeweler’s scale to measure the hair. If scalp hair isn’t available, a similar quantity of body hair might be obtained. When mentioning head hair, it pertains solely to scalp hair. Body hair encompasses all other forms of hair (facial, axillary, etc.).

Process Overview

The laboratory process involves four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step in entering a sample into a lab’s framework. This phase includes checking that the sample was properly sealed and dispatched, generating a random LAN (Laboratory Accessioning Number), and accomplishing any data entry not available through an electronic chain of custody.

Screening serves as a preliminary drug-abuse test. Although Screening is an economical approach to exclude drug use on most samples, a positive finding must be verified for legal acceptance. Any samples yielding a preliminary positive in Screening undergo further confirmation.

If a sample shows a preliminary positive during Screening, additional hair is extracted from the original sample and prepped for Extraction. Here, drugs are isolated from hair at significantly lower concentrations than with other techniques (such as urine or oral fluid), hence performing hair drug screening is notably challenging.

Confirmation of any positive findings from Screening is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with a preliminary positive flag are cleanse before confirmation if necessary. The full lab process from Accessioning through to Confirmation is evaluated under CAP (College of American Pathologists) Hair criteria and the ISO / IEC 17025 accreditations.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
