What it is, How it works

Hair analysis has emerged as a robust method for identifying usage of drugs and alcohol. Hair offers an extended timeline of alcohol and drug consumption by capturing biomarkers within the hair strand's fibers as it grows. When taken near the scalp, hair can represent up to a three-month detection period for both alcohol and drugs. It is straightforward to collect, somewhat resistant to tampering, and convenient to send off.

A segment of hair measuring 1.5 inches from around 200 strands (equivalent to a #2 pencil) drawn from close to the scalp will yield 100mg of hair, which is the optimal amount for both screening and verification. For EtG, additional tests, or tests exceeding 10 panels, a specimen of 150mg is advised. Using a jeweler’s scale for measuring the sample is recommended. In cases where scalp hair cannot be obtained, a comparable amount of body hair will suffice. When we mention head hair, we specifically mean scalp hair only. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result consists of four major steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves bringing a sample into a lab's workflow system. Tasks include confirming that the sample is sealed and transported correctly, assigning a unique LAN (Laboratory Accessioning Number), and completing any additional data entry not covered by an electronic chain of custody system.

Screening is an initial assessment to identify drugs of abuse. Though Screening is cost-efficient in ruling out drug usage in most samples, any positive findings need confirmation for court admissibility. Samples with a presumptive positive in Screening need secondary verification.

If a sample yields a presumptive positive in Screening, additional hair is extracted from the original specimen for the Extraction step. Here, drugs are extracted from hair at far lower concentrations than in alternative methods (e.g., urine or oral fluid), which makes hair drug screening one of the most arduous methodologies.

Positive screening results are validated using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are cleansed before verification where necessary. The entire process from Accessioning through to Confirmation is conducted under the guidance of the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
