What it is, How it works

Hair serves as a comprehensive historical record of alcohol and drugs by encapsulating biomarkers within the fibers of the continuously growing hair strand. When collected near the scalp, hair can offer up to a 3-month detection timeframe for alcohol and drugs. The process of gathering hair is straightforward, challenging to tamper with, and convenient to send.

A 1.5-inch section comprising around 200 strands of hair (resembling the thickness of a #2 pencil) nearest to the scalp supplies 100mg of hair, which is the ideal quantity for initial screening and subsequent confirmation. For EtG, additional tests, and/or assessments exceeding 10 panels, a specimen of 150mg is advised. It is recommended to weigh the specimen using a jeweler's scale. Should scalp hair be unavailable, a comparable quantity of body hair can be taken. When referencing head hair, it pertains solely to scalp hair, while body hair encompasses all other types (such as facial, axillary, etc.).

Process Overview

The laboratory workflow for processing a drug test outcome can be divided into four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial entry of a specimen into a lab's system. This phase includes ensuring the sample was properly sealed and transmitted, assigning a random LAN (Laboratory Accessioning Number), and fulfilling any required data entry absent in an electronic chain of custody framework.

Screening consists of an initial rapid assessment for drugs of abuse. While Screening is a cost-effective mechanism to exclude drug usage for most samples, a positive outcome requires confirmation to be accepted legally. Samples that yield a presumptively positive result during Screening necessitate subsequent confirmation.

In cases where a sample exhibits a presumptively positive result at the Screening phase, additional hair from the original specimen is utilized for Extraction preparation. This phase involves extracting drugs from hair at a significantly lower concentration than other methodologies (e.g., urine or oral fluid), rendering hair drug screening the most challenging methodology to execute.

Verification of any positive screening result is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are subjected to washing before confirmation as necessary. The entire laboratory process, from Accessioning to Confirmation, adheres to both the CAP (College of American Pathologists) Hair designation and the accreditation per ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
