A 1.5-inch segment consisting of approximately 200 hairs (roughly the diameter of a #2 pencil) nearest to the scalp yields 100mg of hair, which is optimal for initial screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, it is advised to collect 150mg of hair. Using a jeweler’s scale to weigh the sample is advisable. If scalp hair is not available, an equivalent quantity of body hair is permissible. When discussing head hair, this refers exclusively to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The process involves four key steps in the lab processing of a drug test result, namely Accessioning, Screening, Extraction, and Confirmation.

In Accessioning, the primary process of integrating a sample into the lab's system occurs. This phase verifies proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes other necessary data entry absent from an electronic chain of custody system.

Screening serves as an initial rapid assessment for drugs of abuse. Though Screening is a cost-efficient approach to eliminate drug use likelihood in most samples, a positive screen necessitates confirmation for legal admissibility. Presumptively positive samples require a subsequent confirmation procedure.

If a sample preliminarily tests positive at the Screening phase, more hair is extracted from the original sample and readied for Extraction. In this step, drugs are removed from hair at concentrations lower than other methods (e.g., urine or oral fluid), rendering hair drug screening the most intricate method to execute.

Positive screening outcomes are confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results undergo preconfirmation washing as necessary. The entirety of the laboratory procedure, from Accessioning to Confirmation, is evaluated under the CAP (College of American Pathologists) Hair designation alongside ISO / IEC 17025 standards accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
