A sample measuring 1.5 inches, consisting of around 200 hair strands (comparable to a #2 pencil width) adjacent to the scalp, will provide 100mg of hair, which is the preferred quantity for analysis and verification. For EtG, supplementary tests, and/or tests exceeding 10 panels, a 150mg sample is advisable. We suggest weighing the specimen using a jeweler’s scale. If scalp hair is unavailable, a similar quantity of body hair is suitable for collection. When referring to head hair, we specifically mean scalp hair. Body hair pertains to all other hair types (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result mainly involves Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab's workflow. This step includes verifying sealing and shipping integrity, assigning a random LAN (Laboratory Accessioning Number), and completing supplemental data entry not covered by an electronic chain of custody system.

Screening is a preliminary assessment for drugs of misuse. Although Screening is an economical technique to exclude drug usage in most cases, a positive screen requires confirmation to be valid in a court setting. Any samples deemed presumptively positive during Screening warrant further confirmation.

When a sample is presumptively positive during Screening, additional hair is extracted from the original specimen for further preparation and Extraction. During Extraction, drugs are isolated from hair in much lower concentrations compared to other methods (e.g., urine or oral fluid), which contributes to the complexity of hair drug screening.

Confirmation for any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumed positive samples are washed as necessary before confirmation. The complete lab process, from Accessioning through Confirmation, is subject to review under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
