Securing a 1.5-inch section of around 200 hair strands (equivalent to a #2 pencil size) adjacent to the scalp will yield approximately 100mg of hair, which is optimal for screening and verification. For EtG, additional, and/or tests over 10 panels, a 150mg sample is advised. We suggest using a jeweler's scale to weigh the sample. If scalp hair is inaccessible, a comparable quantity of body hair may be used. When discussing head hair, we mean scalp hair only. Body hair encompasses all other types like facial or axillary hair.

Process Overview

The laboratory processing of a drug test result entails four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial sample processing into the lab’s system, including validation of sample seal and shipping, assigning a random LAN (Laboratory Accessioning Number), and input of any additional data not captured by an electronic chain of custody system.

Screening is a preliminary assessment for drug presence. Although cost-effective for eliminating drug use in most samples, a positive result in Screening requires verification for court admissibility. Any samples indicating potential positivity during Screening must undergo secondary confirmation.

For samples indicating potential positivity in the Screening phase, additional hair is extracted from the original specimen and prepared for Extraction. This stage involves extracting drugs from hair at significantly lower concentrations compared to alternative methods (e.g., urine or saliva), which contributes to the complexity of hair drug screening.

Any positive result from Screening is confirmed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with preliminary positive outcomes are cleansed before confirmation if necessary. The comprehensive laboratory procedure, from Accessioning to Confirmation, adheres to both the CAP (College of American Pathologists) Hair certification and ISO/IEC 17025 accreditation standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

Novel Analytical Techniques: The Verona meeting witnessed the unveiling of cutting-edge analytical techniques, offering enhanced sensitivity, selectivity, and accuracy for drug testing. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
